Alternative Family Living (AFL)
At Changing Seasons HealthCare, our AFL program has 24/7 specialized care, in a licensed residence, single room occupancy home. Our services are designed to provide family living, in a safe, secure & interactive environment, aligning with your loved one's physical or developmental goals. AFL family living includes personal care assistance, community outings, financial education and inter-personal skill building. Whether it's a Traumatic Brain Injury, Intellectual and Developmental Disabilities (I/DD), Co-occurring Mental Health and Cognitive Conditions, we are here to support you!
Family Support Services
We provide family caregivers respite services while ensuring loved ones continue receiving quality care. This allows caregivers to benefit from our 24/7 specialized care and comprehensive family support services.
Active Community Inclusion
We provide transportation and planning for social clubs, as well as family support services for volunteer opportunities, employment coaching, and recreational activities through our in-home care services.
